Affidavit of Marriage Form

The affidavit of marriage may be used in lieu of a marriage certificate. Form must be notarized to be valid, which may be done in our office, free of charge.

This can be used even when last names do not match. The document is only used in-house as proof of marriage and the Notary Public is not held liable if the parties give false information.
