Landlord Agreement Form

This form should NOT be used by tenants/renters for any reason.  To apply for service as a tenant, complete the New Service Application.

Please note that our residential rate is specifically designed for a member's primary residence and any outbuildings directly associated with that residence, such as garages, barns, or sheds.  Any additional accounts that do not meet this definition, including rental properties or second homes, are subject to our general service/small commercial rate.

*If you have more than three addresses to place under your landlord agreement, you may attach a spreadsheet of all addresses on Page 5.*
